Chain Tensioner Problems

I recently had to take my back wheel off to replace my inner tube, before I did this my chain ran fine over my chain tensioner without any problems. But since I have put me rear tyre back on, the chain seems to be pulling away from the frame on the chain tensioner (note: I have a fixed tensioner). This causes the chain to catch on the rear cog and locks the chain up until it hops down with force. Anyone got any sort of idea what might have happened and how to fix it? Thanks for your help. :)

no a only recently got that rear cog, ill have a look at it, but it barely been riden on. Its strange, when i pedal in the opposite direction, it sits beautifully in the middle of the tensioner wheel, but when i pedal forward, it mores down to the chainstay as if it is tapered down.
