The spacer should go inside the thread of the crank and sit on the bb axle, this is shaped to fit with the part of the extractor that screws in towards the axle and pulls the arm off. So just sit it over the screw-y bit level with the end of the threads on the extractor, screw the extractor into the crank arm and the get a big ass spanner on the end of it to tighten it up, it'll come off no problem. Try and find someone who knows what they're doing. yarp. just take the crank bolt out, put a 5p in its place to cover the hole, screw on the crank extractor.
