What about running a 4 bolt booster under the brake adapters? Steve, I'm trying to figure out the extra mount position... On the top one, the 4 bolt mount would line up with the 3 bottom holes on the adapters... But if you mounted it on the lower hole on the adapter, where would you bolt the bottom of it? I'm probably being stupid (tired!) but I can't see it?

Just got some new Koxx browns, a fresh grind, and oh my god. My brake is amazing! haha. Took my booster off, and it got really loud (as expected) and it held way better. Also got some of those new Try-All mounts, so I don't need the stupid plastic slave washers, and my brake is perfectly flat on the rim. Spent literally 2 minutes setting the brake up! Not the 20odd I used to spend trying to get the pads flat on the rim.

My mates running a v-brake on the front with an onza drilled rim, and the brake's awesome! Doesn't eat the pads either, suprisingly. They're only the standard Avid black pads too...

Tracking isn't just parallel on every car though, some toe in a bit, some toe out. It'll be easier to just go to a garage, and ask to watch them do it (say you're interested or something) and make sure they don't rip you off with it.

As soon as the chain stretches you'd end up needing a tensioner anyway! Get horizontal dropouts, and ask him about the dropout design (onza use it now) which is a bolt that pushes the wheel back. I know he can do that, because I asked him.
